New Open Position: Case Manager

Case Manager – Homeless Outreach (Full-Time 37.5 Hours)

Reports To: Executive Director

Purpose: Provides comprehensive case management to homeless clients including screening, assessment, planning, and evaluation of services. Provides oversight for the outreach and case management team.

GENERAL STATEMENT OF DUTIES

The Case Manager will provide a range of services with the goal of connecting people to shelter and ultimately stable housing along with tailored services and supports of their choice, such as health and behavioral health care, transportation, access to benefits, and more. The Case Manager is expected to actively participate as a member of the outreach team, working both collaboratively and independently to provide screening, and assessment of client situation, advocacy, crisis intervention, basic needs support and case management.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Interview and collect environmental, psycho-social, financial, employment, housing, educational, functional and health information as appropriate to assess client needs and create detailed service plans
  • Work effectively with populations that have experienced vulnerability, trauma, poverty, incarceration, substance use, developmental delays, compromised mental wellness, brain injuries, literacy and numeracy issues, and/or, other conditions or situations that have impacted housing stability
  • Empower clients with accessing benefits and resources such as social, financial, legal, educational/occupational and healthcare services that will stabilize housing and promote well-being
  • Advocate for client interests both orally and through written correspondence
  • Create and maintain case notes for each client and secure documentation required for housing
  • Collect and enter client data into the Homeless Management Information System
  • Maintain confidentiality and respect in all aspects of the work environment
  • Attend and actively participate in all agency/departmental required trainings and/or staff meetings
  • Other duties may be required as time permits, at the discretion of the Executive Director

REQUIRED QUALIFICATIONS

  • Bachelor’s Degree in Social Work or Human Services, or equivalent education and/or experience
  • Knowledge or practice of Intensive Case Management Housing First model of case management
  • An ability to organize, prioritize and perform tasks in an accurate manner with specific attention to detail
  • Proficiency with Microsoft Office and Google Docs
  • Excellent oral and written communication skills

DESIRED QUALIFICATIONS

  • Bilingual (English-Spanish). The ability to read and write in Spanish a plus but not required
  • Maintain a valid driver’s license, and a properly insured, inspected and registered automobile
